SWCHC is humbled to have wonderful partners such as:

Kaiser Permanente and Providence Health System

SWCHC is grateful to Kaiser and Providence for its two year grant (2016-2017) supporting our transition into the Primary Health and Wellness Model. The grant will assist with plan and implementing a service model which offers annual wellness visits and all the recommended preventative screenings to our patients.

Oregon Jewish Community Youth Foundation

OJCYF generously awarded SWCHC funds supporting eye exams and glasses for our patients. Our patients’ safety and quality of life will be significantly improved thanks to proper eye sight afforded by eye exams and glasses provided by The Oregon Jewish Community Youth Foundation.

Legacy Health System

Legacy Health System has been a wonderful supporter of SWCHC for many years. We are truly appreciative of their most recent annual gift supporting our operations. Support from Legacy Health System allows us to “keep the lights on” and serve our patients on a day to day basis.

Providence Health System

SWCHC and Providence Health System enjoy a multifaceted relationship. Providence and SWCHC has a contract regarding referring patients from St. Vincent’s emergency room to SWCHC. Providence also supports SWCHC with regular generous contributions. We are very fortunate to have partners like Providence.

The RE/MAX Equity Group Foundation

The goal of the RE/MAX Equity Group Foundation is to give back to the community served by the Re/MAX Equity Group agents and staff. SWCHC is most thankful to the RE/MAX Equity Group Foundation for a recent grant supporting eye exams and glasses.

Gilead Foundation

SWCHC is especially grateful to Gilead foundation for their award of $20,000 supporting the women’s clinics. These funds will support all preventative tests offered to women as part of their routine medical exam, including PAP, HPV, HIV and Mammograms.

Holzman Foundation

We are proud to partner with the Holzman Foundation for the purpose of promoting human service needs and caring for our community. Thanks to the Holzman Foundation SWCHC is able to offer diabetic patients with eye exams that may prevent blindness.

PacificSource

PacificSource has awarded SWCHC a three year grant (2012-2014) supporting the Clinical Assurance Program which ensures our physicians have the tools they need to diagnose and treat our patients. Thanks to this grant our patients can feel confident that when they schedule an appointment at SWCHC they will receive the care they need. 

Tuality Healthcare

Tuality Healthcare has a special connection with SWCHC-Hillsboro Clinic. Tuality supports SWCHC-Hillsboro clinic by providing in-kind labs and imaging to our patients in Hillsboro. This service is valued at approximately $8,000 per month. SWCHC is also most appreciative of Tuality Healthcare’s generous monetary gifts supporting clinical services.

Episcopal Parish of St. John the Baptist

Episcopal Parish of St. John the Baptist raised $15,000 for SWCHC in a auction fundraising event this July. According to leaders at the parish, this is the largest amount they have ever raised for an organization. We are grateful to the parish and all its members for their compassion for the sick and for their generosity. 

The Standard

The Standard contributed $2,500 to SWCHC to support general operations in honor of The Standard employee, Alison Daily, who is currently serving as SWCHC’s board chair.

First Unitarian Church of Portland

SWCHC is grateful to First Unitarian Church of Portland for a “split the basket” gift. The church elected to share half the proceeds from its collection plate in the month of July with SWCHC.

The Reser Family Foundation

SWCHC is most appriciatove of its ongoing relationship with the Reser Family Foundations. In July 2014 we were grateful to receive a grant supporting our clinical services and targeted specifically to eye exams and imaging tests.

SW Hills Kiwanis Club

SW Hills Kiwanis Clubs hosts an annual pancake breakfast fundraiser for SWCHC. This year’s event, held in May, raised $4,850. This group is truly inspirational. Every year we look forward to working together on this event, and every year we are humbled by their effort and the results they produce. Thank you SW HIlls Kiwanis!
