7 Years of Service

In May 2012, SWCHC celebrated its 7th anniversary. Starting out as a clinic, located in the Multnomah Center and only open 2 nights a week, SWCHC moved to its present location - 7754 SW Capitol Highway - in 2009. We currently offer clinics every Monday, Tuesday and Thursday as well as the first and second Wednesdays of each month.

Over the past year, SWCHC provided its 10,000th patient visit (see 10 Steps to 10,000 Visits) and continues to move forward in providing high quality acute/episodic care, preventive health services and chronic disease management. We recently began diabetic group visits to enhance diabetes self-management through clinical care and peer support.

We believe a healthy community is a strong community - and put that belief into practice with the provision of quality, culturally-competent care.

Join us in supporting access to quality care. A donation of:

$144 – pays for a patient visit
$90 – pays for one month of diabetic supplies
$35 – pays for a complete lab panel
